Promoting health and identifying symptoms of both acute and chronic illnesses are paramount to the well-being of all individuals. Hypertension, diabetes, and cardiovascular disease are often left untreated due to the absence of symptomatology. Navigational barriers in healthcare access, can, and often do, lead to life-threatening situations. Our Wellness Center tackles these barriers by providing one-on-one health checks and medical referrals for follow-up care. When necessary, emergency medical treatment is coordinated for immediate care. The Wellness Center nursing team will coordinate with primary and specialty care physicians to assist individuals with prompt medical treatment.
The integration of primary care and behavioral health services was developed to better address the physical healthcare needs of individuals with mental health and addiction disorders. Removing complex navigational barriers creates better access and improved outcomes. Primary Care services are provided on-site by area medical professionals.
Comprehensive laboratory services for testing and screening makes it convenient for individuals to complete their lab orders on-site. Services are available 5 days per week and no appointment is necessary.
